1. This practice charges private fees and tariffs, which may exceed the standard fees, used by Medical Funds or Schemes.

  2. Some of the procedures are not covered by the benefits provided by Medical Schemes or Funds.

  3. Should you be a member of a Medical Scheme or Fund, even if the scheme has extensive dental benefits, does not mean that your Medical Fund / Scheme will accept any claim for the procedures provided.

  4. Should you wish to claim a benefit under your Medical Scheme, it isyour exclusive responsibility to obtain the information and procedures to follow to obtain authorization or to claim any benefit.

  5. YOU ARE PERSONALLY LIABLE FOR PAYMENT OF ALL FEES, ON ACCEPTANCE OF THE WRITTEN QUOTATION, AND THE FULL BALANCE ON COMPLETION OF WORK.

  6. Should you fail to make any payment in full on due date, the whole amount will immediately become due and payable.

  7. You are not allowed to withhold payment for whatever reason.

  8. Should any dispute regarding services arise, you are obliged to make full and timeous payment into the Attorney’s Trust account nominated in writing by Dentistry SA, to be held and invested in trust to Section 78(2)(A) of the Attorneys Act pending resolution of the dispute.

  9. A 3% interest per month will be charged on outstanding accounts older than 60 days.

  10. You will be liable for legal fees on an attorney – own client scale, tracing fees and collection, should you fail to make any payment timeously.

  11. A claim against the Medical Scheme is a contractual relationship between my Medical Scheme and me and not between Dentistry SA and the Medical Scheme.

  12. With agreement of the practice terms you concent of the practiceI consent to the jurisdiction of the Magistrates Court for any action arising from this agreement or any service rendered by Dentistry SA.
